Deputy Denis Naughten asked the Minister for Health following the publication of the health care quality indicators in the Irish health system report last week, if he will outline the data outliers which gave rise to concerns regarding patient safety; the date on which his Department became aware of each safety concern; the date on which the Health Service Executive was informed of each safety concern and the corresponding date on which each hospital was contacted; the date on which his Department received a response back from either the HSE or hospital concerned; the follow up actions taken in each case; the date when each safety concern was addressed to the satisfaction of his Department; and if he will make a statement on the matter.
