Deputy Pearse Doherty asked the Minister for Health the reason a transitional care plan compiled in respect of a person (details supplied) outlining the individual care needs of that person was not implemented by a training centre for adults with disabilities in County Donegal; if an individualised care plan was ever created for the person while the person was a service user at the aforementioned facility; if so, if he will make this plan available to the person for the person's information; and if he will make a statement on the matter.
