Deputy Aengus Ó Snodaigh asked the Minister for Health the reason the documents relating to a medical card application in respect of a person (details supplied) in Dublin 10 have been mislaid several times and the person has been provided a new application number on the pretext that the required medical and financial documents were not supplied in the required time despite the documents having been sent in, which does not take account of the date on which the first application was made and starts only from December 2014; and the action she will take to re-instate the original application date and have a medical card issued forthwith.
