Summary
Medicaid Fraud Prevention Act of 1995 - Amends title XIX (Medicaid) of the Social Security Act to require as a condition of receiving payments for administration costs that a State include on the Medicaid enrollment card provided to beneficiaries under the plan a photograph of and specified information about the beneficiary, unless such a practice is not cost-effective because minimal fraud exists in such State as a result of incorrect identifications.
