Summary

The purpose of the bill is to link the currently separate healthcare and social systems into a more unified whole and to improve the organization of assistance for people who need support from both areas at the same time. Currently, health care is mainly the responsibility of the state and the Health Fund, as well as social services of local governments, which is why different work arrangements and insufficient data exchange can cause aid to be delayed or duplicated. Population also increases the need for changes ageing, increasing chronic diseases and rising costs. With the reform, county welfare regions and cooperation groups will be created, as well as a human-centered cooperation organization in the field of health and social care, or TERVIK, in each region. It unites local governments, family doctors and hospitals into a cooperative network working towards common goals. With the help of the health and well-being profile, the needs of the residents of the region will be linked with the use of services and with costs. The role of the Health Fund extends from financing treatment to supporting regional cooperation, prevention and health promotion. A coordination service and the role of a health guide will be created for people with complex service needs. The health guide prepares a well-being plan for the person, gathers information from the health and social fields and coordinates the necessary services and information exchange. In the start-up phase, the reform will increase the capacity of local governments and health and the workload of social workers, but in the longer term, a unified work organization with a coordination service and digital solutions should reduce duplication and time spent by specialists. Regional cooperation organizations will be established by November 1, 2027, and performance funding and coordination service will be implemented from May 1, 2028. The estimated total cost of the reform in the years 2026-2032 is nearly 64 million euros, which will mainly finance the service of health guides, management of cooperation organizations, IT solutions, trainings and additional workforce of the Health Insurance Fund. By 2030, approximately 231 specialists are needed, who are planned to be prepared gradually.
