Summary
Eelnõu eesmärk on siduda praegu eraldi toimivad tervishoiu- ja sotsiaalsüsteem ühtsemaks tervikuks ning parandada abi korraldamist inimestele, kes vajavad korraga mõlema valdkonna tuge. Praegu on tervishoid peamiselt riigi ja Tervisekassa ning sotsiaalteenused kohalike omavalitsuste vastutusel, mistõttu erinev töökorraldus ja puudulik andmevahetus võivad põhjustada abi hilinemist või dubleerimist. Muudatuste vajadust suurendavad ka rahvastiku vananemine, krooniliste haiguste sagenemine ja kasvavad kulud. Reformiga luuakse maakondlikud heaolupiirkonnad ja koostöökogud ning igasse piirkonda tervishoiu- ja sotsiaalvaldkonna inimesekeskne koostööorganisatsioon ehk TERVIK. See ühendab kohalikud omavalitsused, perearstid ja haiglad ühiste eesmärkide nimel tegutsevaks koostöövõrgustikuks. Tervise- ja heaoluprofiili abil hakatakse siduma piirkonna elanike vajadusi teenuste kasutuse ja kuludega. Tervisekassa roll laieneb ravi rahastamiselt ka piirkondliku koostöö, ennetuse ja tervise edendamise toetamisele. Kompleksse teenusevajadusega inimestele luuakse koordinatsiooniteenus ja terviseteejuhi roll. Terviseteejuht koostab inimesele heaoluplaani, koondab tervise- ja sotsiaalvaldkonna info ning koordineerib vajalikke teenuseid ja infovahetust. Reform suurendab käivitusfaasis kohalike omavalitsuste ning tervishoiu- ja sotsiaaltöötajate töökoormust, kuid pikemas vaates peaks ühtne töökorraldus koos koordinatsiooniteenuse ja digilahendustega vähendama dubleerimist ning spetsialistide ajakulu. Piirkondlikud koostööorganisatsioonid luuakse 1. novembriks 2027 ning tulemusrahastamine ja koordinatsiooniteenus rakenduvad 1. maist 2028. Reformi hinnanguline kogukulu aastatel 2026–2032 on ligi 64 miljonit eurot, millest rahastatakse peamiselt terviseteejuhtide teenust, koostööorganisatsioonide juhtimist, IT-lahendusi, koolitusi ja Tervisekassa lisatööjõudu. 2030. aastaks on vaja ligikaudu 231 spetsialisti, keda plaanitakse ette valmistada järk-järgult.
