Maritime Safety Act Amendment Act

The bill amends the regulation of waterway fees in the Maritime Safety Act. The amendments establish waterway fee discounts based on the ships' environmental performance in order to encourage the use of environmentally friendly ships and investments in green technologies. The unit price of cruise ships and the upper limit of the waterway fee paid for one ship visit will be increased to harmonize them with the fee rates of other ship types. In addition, the exception according to which there was will be abolished bunkering in the anchorage area is exempt from paying the waterway fee. Since it may be an environmentally hazardous activity, maintaining the exemption is not justified. At the same time, the exemption from the waterway fee remains valid for the bunkering of alternative and renewable fuels.

Act on Amendments to the Public Health Act and Amendments to Other Acts Related to It (Amendment of the Rehabilitation System)

The purpose of the bill is to create a person-centered rehabilitation system, where the person's ability to function and the need for help are comprehensively assessed and the necessary help is available in a timely manner. The goal is to simplify a person's journey to help, align the content of the service with the person's actual need for help, and reduce duplication in the healthcare and social system. The changes support the development of the health care system towards the holistic well-being of the person - it is not only important treatment, but also restoration, preservation and improvement of functioning capacity at all stages of life. Current rehabilitation services are fragmented between the social and health systems, and access to services has depended on formal assessments rather than actual need. Services with a similar content are offered in two areas, and their financing and organization takes place on different bases. According to the bill, the rehabilitation service will be integrated more closely than before with the health care system, both in primary health care and in hospitals. Rehabilitation services will be purchased through the Health Fund in coordination with healthcare services. This makes it possible to create target group-based rehabilitation teams, reduce bureaucracy, use specialist resources more effectively and ensure a unified and logical service system for the person.

Act to amend the Natural Gas Act and Tax Administration Act

The bill creates a legal framework in Estonia for reducing methane emissions from the energy sector and adopts the requirements of the corresponding regulation of the European Union. The Environmental Board is appointed as the competent authority, which will check compliance with requirements, carry out supervision and inspections, and cooperate with other member states and the European Commission. Liability for violating the requirements of the regulation is also established. The amendments are introduced for gas network operators stricter requirements for measuring and reporting methane emissions and detecting and remediating methane leaks. It is also necessary to start reporting more precisely on methane emissions related to imported fossil fuels. The goal is to reduce strong greenhouse gas emissions and contribute to the achievement of the climate goals of Estonia and the European Union. The bill increases the administrative burden of gas network operators and adds new tasks to the Environmental Board. If to fulfill them no additional money is allocated from the state budget, the costs are covered from the existing funds of the ministry.

Value Added Tax Act Amendment Act

The bill adopts the amendments to the European Union's VAT directive, which aim to reduce the administrative burden on businesses operating across borders. The amendments expand the possibility of using the special procedure for distance sales (OSS), so that the entrepreneur can fulfill the VAT liability arising in other member states through his country of residence and does not have to register as a VAT payer in each country.

The Act to amend the Income Tax Act and other related laws

The bill simplifies and modernizes the submission of tax returns in order to reduce both the administrative burden of taxpayers and the workload of the state. Instead of the previous static declaration forms, lists of data to be submitted will be stipulated in future laws, which will enable the Tax and Customs Board to design the e-environment more flexibly and develop automatic data exchange between economic software and MTA systems. Changes are limited submission of paper declarations. Employers must in future submit the TSD declaration electronically regardless of the number of employees. Until now, it was possible to submit a declaration on paper for up to five employees. In the event of an objective obstacle, the possibility remains to request a paper submission permit.

Act on Amendments to the Public Health Act and Amendments to Other Acts Related Thereto (Cooperation in the Health and Social Sectors)

The purpose of the bill is to link the currently separate healthcare and social systems into a more unified whole and to improve the organization of assistance for people who need support from both areas at the same time. Currently, health care is mainly the responsibility of the state and the Health Fund, as well as social services of local governments, which is why different work arrangements and insufficient data exchange can cause aid to be delayed or duplicated. Population also increases the need for changes ageing, increasing chronic diseases and rising costs. With the reform, county welfare regions and cooperation groups will be created, as well as a human-centered cooperation organization in the field of health and social care, or TERVIK, in each region. It unites local governments, family doctors and hospitals into a cooperative network working towards common goals. With the help of the health and well-being profile, the needs of the residents of the region will be linked with the use of services and with costs. The role of the Health Fund extends from financing treatment to supporting regional cooperation, prevention and health promotion. A coordination service and the role of a health guide will be created for people with complex service needs. The health guide prepares a well-being plan for the person, gathers information from the health and social fields and coordinates the necessary services and information exchange. In the start-up phase, the reform will increase the capacity of local governments and health and the workload of social workers, but in the longer term, a unified work organization with a coordination service and digital solutions should reduce duplication and time spent by specialists. Regional cooperation organizations will be established by November 1, 2027, and performance funding and coordination service will be implemented from May 1, 2028. The estimated total cost of the reform in the years 2026-2032 is nearly 64 million euros, which will mainly finance the service of health guides, management of cooperation organizations, IT solutions, trainings and additional workforce of the Health Insurance Fund. By 2030, approximately 231 specialists are needed, who are planned to be prepared gradually.

Act on the Amendment of the Act on the Organization of Health Care Services and the Act on the Organization of the Defense Forces (amendments related to business license and registration)

The purpose of the bill is to update the organization of healthcare services, strengthen patient safety and give the Health Board better opportunities to assess the quality of healthcare services. In addition to the formal requirements, when granting an activity license, the Board of Health will be able to assess the content of the service, work organization and quality and patient safety assurance. At the same time, the activity license procedure is simplified, using more of the data already available in the registers and reducing it duplicative administration. The grounds for suspending the registration of a healthcare worker are also specified. The Board of Health can intervene if a healthcare worker significantly or repeatedly violates professional or professional requirements, or his actions threaten the patient's safety or the quality of treatment. Before the suspension of the registration, as a milder measure, it is possible to require the completion of additional training, and to restore the suspended registration, a theory and practical exam must be successfully completed. Also the organization of the ambulance service is specified: the ambulance is more clearly linked to the outing organization of the Emergency Center and is distinguished from ordinary patient transport and other support services. The provision of healthcare services in a crisis situation will be made more flexible. In coordination with the Health Board, if necessary, the place of service provision can be changed, the service can be provided without a normal operating license, and foreign armed forces and voluntary associations can be involved in the provision of assistance. When applying for an activity license, the administrative burden on entrepreneurs may initially increase somewhat, but digitization and greater use of register data should ultimately reduce this. In addition, the organization of the ambulance service will be specified and the possibility of using special-purpose brigades will be created. From now on, the assistance provided on the basis of the outing order of the Emergency Center will be considered ambulance, which helps to distinguish it more clearly from, for example, patient transport and events of medical security. At the same time, hospitals can use off-duty teams for special purposes, such as an ambulance to transport patients in critical condition from one hospital to another. In the future, the rules of family medical care will not be applied to the healthcare of the Defense Forces, and in a crisis situation, doctors and nurses from foreign countries and international voluntary medical organizations can be more flexibly involved in the provision of healthcare services.
