Summary
Eelnõu eesmärk on luua õiguslikud alused alates 2020. aastast arst-residentide tööjõukulu hüvitamiseks Eesti Haigekassa eelarvest. Eelnõu kohaselt rahastatakse residentuuri korraldamise, teoreetilise koolituse ja residentuuri baasasutuse korralduskulusid riigieelarvest. Arst-residendi tööjõukulud hüvitab edaspidi haigekassa. Seni on kõik residentuuri kulud rahastatud riigieelarvest Sotsiaalministeeriumi kaudu. Põhimõttelise otsuse arst-residendi tööjõukulude hüvitamiseks alates 1. jaanuarist 2020. a haigekassa kaudu tegi Vabariigi Valitsus juba 21. septembri 2017. a istungil, kui vaadati üle haigekassa tulubaasi laiendamise võimalused. Valitsus otsustas etapiviisiliselt viia osa riigieelarvest rahastatavaid teenuseid haigekassasse, et vähendada rahastamise killustatust ja seeläbi suurendada tervishoiusüsteemi tõhusust. Näiteks otsustati 2018. aastal anda riigieelarvest rahastatavatest teenustest haigekassale üle hambaravi toetuse, kiirabi osutamise, meremeeste ööpäevaringse eesti- ja ingliskeelse meditsiinilise kaugkonsultatsiooni võimaluse tagamise, täiendava ravimihüvitise ja perearstide asendustasude rahastamise kohustus. Alates 2019. aastast otsustati Eesti Haigekassa kaudu rahastada HIVi ravimeid, immuunpreparaate, ravikindlustamata isikute vältimatut abi, tuberkuloosiravimeid ning antidoote. Asjakohased seadusemuudatused on Riigikogu ka vastu võtnud. Eelnõuga kavandatud muudatused jäid tookord tegemata, sest vajasid nii haigekassa kui tervishoiuteenuse osutajatega läbirääkimist. Kavandatud muudatused puudutavad eelkõige Eesti Haigekassat, kes hakkab Sotsiaalministeeriumi asemel hüvitama Tartu Ülikoolile arst-residentide tööjõukulu. 2020. aastal on selleks kavandatud 18 miljonit eurot. Samuti puudutab eelnõu Tartu Ülikooli, kellele lisandub Sotsiaalministeeriumi kõrval täiendav residentuuritellimust rahastav lepingupartner Eesti Haigekassa.
