Summary
Mrs. Cécile Muschotti alerts the Minister of Solidarity and Health about the future of the maternity ward at Hyères hospital. The information on the future of maternity that circulated at the beginning of September 2018 is very contradictory. Between the words of the mayor of Hyères and those of other elected officials, those of the director of Sainte-Musse, and those of the union organizations of the Saint-Tropez health center, Gassin maternity ward, it is difficult to navigate, especially for public opinion! The starting point of this imbroglio is the finalization, at the end of its consultation phase, of the Regional Health Plan, a document which sets out in more than 500 pages the health future of the region, discipline by discipline, for the period 2018-2023. The legal form of the document is very precise; and in addition to the decrees which stipulate it, it must respect an important jurisprudential decision and precisely, a decision of the administrative court of Marseille, rendered on July 2, 2013, annulling paragraphs of a health planning document on the grounds “that it designated a precise geographical location (a city) to set a quantified objective of care provision when only the territory could be the place to do this. From now on the objectives are therefore listed in the ARS documents by territory (the department, for us therefore the Var) and by discipline, emergencies, geriatrics, psychiatry, follow-up and rehabilitation care (SSR) etc. And therefore, with regard to maternity, as Gassin union representatives rightly observed on page 286, the number of maternity sites will drop by 2023 from 7 to 5 and each actor, particularly in the (numerous) consultation meetings with user representatives, knew long ago put names to the two sites that were supposed to disappear. Naturally this legal requirement is a dream means for the leaders of the ARS to carry out a delaying strategy: from expert report to consultation meeting, time passes and resignation gradually gains the morale of users who, in fact, are, in France, very little mobilized on the subject of health (health democracy is in its infancy). So if nothing is happening, the reduction to 5 birthing sites in the Var is a highly probable hypothesis, in any case already included in the planning documents for 2023. This strategy carried out throughout the country is based for maternity on a decree of October 10, 1998 which had marked, at the time, a great advance in the field of health planning, a clarification that few other disciplines medical doctors have since been able to match: the delivery sites are classified into three categories, 1, 2, 3 and the parturients are oriented according to the risk assessed during pregnancy in one or the other of the structures (example of gradation of access: Gassin, Fréjus, Nice). Without entering into scientific considerations, we can consider that this text was a major contribution to public health by thinking, ahead of time, of the course of patients, but that it has since aged (times change even for births...) and that it does not take into account the question of transport, in particular the saturation of road and motorway networks (note that in Hyères half of the births concern the neighboring municipalities Le Lavandou, La Londe, Bormes, Pierrefeu etc.). She would like to know what measures the Government intends to take in this direction.
