Summary

Faced with the problems encountered by patients, the Ministry of Health wanted to rely on the expertise of the National Institute of Health and Medical Research (INSERM) in order to have up-to-date knowledge from international and multidisciplinary research on fibromyalgia. This expertise will allow a referral to the High Authority for Health (HAS) to update the recommendations professional findings made in 2010. Patient associations and experts were interviewed by members of the working group led by INSERM. The publication of the final report is scheduled for the end of March 2019. Furthermore, the main advances made within the framework of the proposals made in 2016 in the report on fibromyalgia submitted to the President of the National Assembly are as follows: - Posted online by the fund national health insurance, on August 29, 2017, of a file on fibromyalgia covering in 4 sheets the definition and causes, symptomatology and diagnosis, treatment and living with fibromyalgia. This document is a support tool for health professionals including general practitioners, medical advisors and doctors from departmental homes for disabled people (MDPH). This file also concerns the knowledge of professionals and patients a “FiRST” self-questionnaire for diagnosing fibromyalgia. Fibromyalgia, however, remains a diagnosis of exclusion in the absence of objective clinical, biological and radiological abnormalities which would confirm the diagnosis. In addition, the FIQ (Fibromyalgia Impact Questionnaire) and revised FIQ screening tests are two tests that have already been included in the guidance report of recommendations for good practice “Adult Fibromyalgia Syndrome” from the HAS in 2010. - Establishment, in April 2017, of a specialized transversal training course (FST) “Pain Medicine” which complements a specialized studies diploma (DES). - Posting online on the website of the Ministry of Solidarity and Health and the French Society for the Evaluation and Treatment of Pain (SFETD), a card interactive chronic pain structures, accessible to health professionals and the public. A “skip the line” experiment for easier access to these structures is underway. - Inclusion in the HAS work program of “recommendations relating to the standard process for the management of chronic pain patients for optimal collaboration between the city and referral structures” in order to help patients healthcare professionals to structure the journey of the patients concerned. The publication of these recommendations is planned for the 4th quarter of 2018. For treatment with repetitive transcranial magnetic stimulation (rTMS), it is necessary to establish long-term data on large cohorts in France. As such, French research programs are currently underway in various evaluation and treatment of pain (30-40% of patients are sensitive to it in studies carried out in other countries).
