Paperwork Reduction in Health Care Act of 1991 - Requires the Director of the Office of Management and Budget to: (1) identify, inventory, and assess the Federal paperwork burden associated with health care services; and (2) establish a goal for reducing such burden in each of FY 1992 through 1997 by at least five percent of the preceding fiscal year's paperwork burden.

Medicare Preventive Benefits Act of 1991 - Amends part B (Supplementary Medical Insurance) of title XVIII (Medicare) of the Social Security Act to provide for coverage of fecal-occult blood tests (FOBTs) and screening flexible sigmoidoscopies for the early detection of colorectal cancer. Covers FOBTs on an annual basis for individuals aged 50 or over. Covers screening sigmoidoscopies provided every five years for individuals aged 50 or over. Provides payment for FOBTs under the laboratory fee schedule, subject to a five dollar limit in 1992. Provides payment for screening sigmoidoscopies under the relative value scale in 1992. Permits the Secretary to modify the frequency criteria after 1994. Provides for Medicare coverage of annual influenza vaccinations and for tetanus-diptheria vaccinations administered every ten years. Provides for Medicare coverage up to age seven according to a frequency schedule to be established by the Secretary of Health and Human Services (HHS) in consultation with appropriate entities. Provides for Medicare coverage of annual screening mammography for the early detection of breast cancer for women over age 64. (Currently, such women are covered for such screening performed every other year.) Requires HHS demonstration projects for the coverage of additional specified preventive care services and reports to specified congressional committees on such projects. Authorizes appropriations. Authorizes an Office of Technology Assessment study and report to the Congress on the development of a process evaluating possible Medicare coverage of other preventive care services.

National Child Abuser Registration Act of 1991 - Authorizes a State child abuse information repository (State repository) to report child abuser information to the National Crime Information Center. Directs the Attorney General to establish guidelines for the reporting of such information. Specifies that such guidelines shall require that: (1) a reporting State ensure that reports of all convictions under the criminal child abuse law of the State are maintained by a State repository; and (2) a State repository maintain close liaison with the National Center on Child Abuse and Neglect and the National Center for Missing and Exploited Children for the exchange of information and technical assistance in cases of child abuse. Requires the Attorney General to publish an annual statistical summary of the child abuser information reported under this Act. Makes compliance with this Act a condition to the receipt by a State of any grant, cooperative agreement, or other assistance under the Victims of Crime Act and the Child Abuse Prevention and Treatment Act.

Provides that for purposes of determining the minimum allocation paid to a State under Federal-aid highway provisions and the amount of any other allocation or appointment of Federal-aid highway funds, the amount of taxes treated as paid into the Highway Trust Fund with respect to alternative sources of energy shall be determined as if such energy sources were taxed as gasoline.

Leading Employers Into Apprentice Partnerships Act - Amends the Internal Revenue Code to provide a tax exemption for organizations administering a qualified apprenticeship education program. Allows a general business credit of 20 percent of contributions made to such programs by a business.

Expresses the sense of the House of Representatives that: (1) U.S. businesses engaged in the rebuilding of Kuwait should, to the maximum extent possible, use U.S. subcontractors and U.S. goods and services; and (2) the Department of Commerce should monitor and encourage the implementation of this policy.

Designates August 1, 1991, as Helsinki Human Rights Day. Authorizes and requests the President to: (1) reassert American commitment to the Helsinki Accords; (2) raise the issue of noncompliance with such Accords with any signatory nation which may be in violation; (3) convey to all signatories of such Accords that respect for human rights and fundamental freedoms is vital to progress in the ongoing Helsinki process; and (4) develop new proposals to advance the human rights objectives of the Helsinki process, including the self-determination of peoples.

Farm Animal and Research Facilities Protection Act of 1991 - Amends title XIV of the Food Security Act of 1985 to add a new subtitle prohibiting (and establishing criminal penalties for) specified acts of destruction, theft, or damage to farm animal, research, and educational facilities. Directs the Secretary of Agriculture and the Attorney General to jointly conduct a study of the extent and effect of terrorism on animal research, production, and processing facilities. Grants jurisdiction for cases arising under these provisions to U.S. district courts. Establishes a private right of action.

Beginning Farmer and Rancher Credit Act of 1991 - Amends the Consolidated Farm and Rural Development Act to direct the Secretary of Agriculture to establish a farm ownership down payment loan program. Provides for: (1) Federal-State assistance coordination for beginning farmers and ranchers; and (2) the establishment of an Advisory Committee on Beginning Farmers and Ranchers. Targets farm ownership loans, loan guarantees, and interest rate assistance to beginning farmers and ranchers. Reduces maximum amounts for farm ownership and operating loans and loan guarantees.

Rural Clinical Laboratory Personnel Shortage Act - Amends the Public Health Service Act to establish a program of contracts under which allied health professionals agree to serve in technical positions as employees in clinical laboratories that serve rural areas with shortages of employees in such positions in consideration of the Federal Government paying a limited amount of the principal and interest of the educational loans of the professionals. Authorizes appropriations. Mandates grants and contracts for the education and training of clinical laboratory personnel. Authorizes appropriations. Requires a program for the accreditation of entities engaged in the certification of clinical laboratory personnel. Provides, until establishment of accreditation standards, for a competency-based examination to determine the proficiency of individuals who do not meet formal educational qualifications.

Hospital Antitrust Fairness Act - Exempts from the antitrust laws combinations of, or contracts to allocate hospital services by, two or more hospitals if: (1) each such hospital is located outside of a city or in a city of less than 125,000 inhabitants; (2) in the most recently concluded calendar year, each such hospital received more than 40 percent of its gross revenue from payments made under Federal programs; and (3) there is in effect with respect to each such hospital a certificate issued by the Health Care Financing Administration (HCFA) specifying that HCFA has determined that Federal expenditures would be reduced, and consumer costs would not increase, if such hospitals merge or allocate the services specified in the request.

Pledge of Allegiance Commemorative Coin Act - Establishes the Pledge of Allegiance Commemorative Coin Advisory Board to: (1) consult with the Secretary of the Treasury (the Secretary) regarding coin design; (2) oversee the expenditure of funds provided to the U.S. Capitol Historical Society for assisting its educational programs and providing non-operating improvements to the Capitol; and (3) establish rules and operating procedures to implement this Act. Directs the Secretary to issue five-dollar gold coins, one-dollar silver coins, and half dollar clad coins emblematic of the Pledge of Allegiance to the Flag. Declares a general waiver of public contract procurement regulations governing coin mintage under this Act. Mandates that surcharges be distributed equally between the Treasury and the U.S. Capitol Historical Society. Provides for audits by the Comptroller General. Requires that: (1) all amounts received from the sale of coins be deposited in the coinage profit fund; and (2) the Secretary take actions to ensure that coin mintage and issuance under this Act will not result in any net cost to the Federal Government.

Expresses the sense of the House of Representatives that the President should: (1) expeditiously complete review of the request of the Soviet Union for $1,500,000,000 in agricultural credit guarantees; (2) carefully evaluate the ability of the Soviet Union to service the debt associated with such request and evaluate such request on a commercial basis in accordance with the Agricultural Trade Act of 1978; (3) investigate every opportunity to grant export credit guarantees directly to individual Soviet Republics if the President can determine that such Republics can adequately service debt associated with the sale of agricultural commodities; (4) continue to impress upon the Soviet Government that its treatment of individual Republics will have an impact on U.S.-Soviet relations, including decisions on whether to extend agricultural credit guarantees; (5) impress upon such Government that the food and feed purchased through credit guarantees shall not be misused to support the military, security, or Communist Party apparatus at the expense of the Soviet people, to pressure the Baltic states or other Soviet Republics to support the "Union Treaty," or for any other coercive or political purpose; (6) secure renewed Soviet assurances to fulfill its commitments under the existing U.S.-Soviet Long-Term Agreement on grains and to repay credits extended under U.S. guarantees according to agreed-upon schedules by acceptable currency or barter arrangements; and (7) approve the Soviet request if the President finds that the Soviet Union meets criteria under such Act.

Income-Dependent Education Assistance Act of 1991 - Establishes the income-dependent education assistance (IDEA) program of supplemental direct higher education student loans in which a borrower's annual repayment obligation is dependent upon both postschool income level and borrowing history. Title I: System for Making Income-Dependent Education Assistance Loans - Directs the Secretary of the Treasury to: (1) make IDEA loans to eligible students in accordance with this title; and (2) establish an account for each IDEA loan borrower and collect repayments on such loans using the income tax collection system under specified Internal Revenue Code provisions added by title II of this Act. Sets forth provisions for the terms and enforcement of IDEA program agreements between the Secretary of Education and eligible institutions. Sets forth provisions for the amounts and terms of IDEA loans. Sets annual limits on the amounts of such loans to various categories of students. Sets a limitation on individual borrowing capacity, with adjustments for inflation and with consideration of any outstanding student loan obligations. Limits the duration of individual eligibility for such loans. Sets forth requirements for: (1) agreements to the terms of such loans; and (2) disbursement of the proceeds of such loans. Sets forth the responsibilities of eligible institutions and of the Secretary of the Treasury for certain information requirements relating to the IDEA loan program. Sets forth provisions for interest charges on such loans. Requires such charges to be added to the recipient's obligation account at the end of each calendar year. Bases such charges on an interest rate equal to the lesser of: (1) ten percent; or (2) the sum of the average bond equivalent rates of 91-day Treasury bills auctioned for the previous year, plus two percentage points, rounded to the next higher one-eighth of one percent. Provides for conversion and consolidation of certain other types of student loans as IDEA loans. Authorizes the Secretary of the Treasury, upon request of borrowers who have received federally insured or guaranteed loans under specified provisions of the Higher Education Act of 1965 (HEA) (Stafford loans) or of the Public Health Service Act (PHSA) (HEAL loans), to make new IDEA loans to such borrowers which are equal to the sum of the unpaid principal of those other loans and which discharge the liability on those other loans. Provides for mandatory conversion of certain defaulted student loans to IDEA loans. Requires IDEA collection treatment of any loan made, insured, or guaranteed under specified provisions of HEA or PHSA after enactment of this Act which is assigned after default for collection by the Secretary of Education or the Secretary of Health and Human Services. Directs such Secretaries to notify: (1) the Secretary of the Treasury of the need to establish or adjust an IDEA account for such loan's borrower; and (2) the borrower of the conversion of the defaulted loan to an IDEA loan and the procedures for collection under the income tax system. Terminates the authority to make additional loans under the HEA programs of supplemental loans for students (SLS) and direct loans to students in institutions of higher education, for any academic year beginning after the date regulations are prescribed by the Secretaries of Education and the Treasury to carry out this title. Authorizes appropriations to: (1) make distributions of IDEA loan funds to eligible institutions; and (2) administer and carry out this title. Bases student eligibility for IDEA loans on their eligibility for student assistance under specified HEA provisions and their carrying at least one-half the normal full-time academic workload. Title II: Collection of Income-Dependent Education Assistance Loans - Amends the Internal Code to add provisions for the collection of IDEA loans. Directs the Secretary of the Treasury to notify each IDEA loan borrower of their yearly repayment obligation. Sets forth formulas for computation of the annual IDEA loan repayment amount. Makes such amount equal to the lesser of: (1) 20 percent of the excess of the modified adjusted income of the taxpayer for such taxable year over the standard deduction and exemption (twice for joint returns); or (2) the product of a base amortization amount and a progressivity factor based on the taxpayer's modified adjusted gross income. Defines "base amortization amount" as the amount which, if paid at the close of each year for 12 consecutive years, would fully repay (with an eight-percent annual interest rate) the maximum account balance of the borrower. Sets forth progressivity factor tables for various types of taxpayers. Provides that, in general, the repayment obligation of an IDEA loan borrower shall terminate only if there is repaid: (1) in the case of any repayment during the first 12 years for which the borrower is in repayment status, the principal plus interest at an annual rate equal to the otherwise applicable rate plus two and one-half percent; and (2) in the case of any repayment during any subsequent year (or in the case of loans under $3,000 repaid during the first 12 years), the principal plus interest at applicable rates. Requires no repayment after 25 years in repayment status. Sets forth provisions for the determination of years in repayment status. Sets forth the requirements for payment of the amount owing. Directs the Secretary of the Treasury to assess and collect any unpaid amount in the same manner as for any delay in the payment of income tax. Provides for discharge, by the Secretary of Education, of the IDEA loan liability of any borrower who dies or becomes permanently and totally disabled. Provides for crediting of amounts paid on a joint return. Sets forth formulas for computation of an alternative annual payment for individuals who have attained age 55. Provides for inflation adjustment in the computation of the progressivity factor. Provides that, in general, an IDEA loan shall not be dischargeable in a case of bankruptcy, but authorizes the Secretary of the Treasury to postpone certain portions of the loan liability in such cases. Makes specified provisions relating to finality of assessment and collection applicable to such loans. Includes the amount required to be repaid for IDEA loan uner amounts listed under requirements relating to failure by an individual to pay estimated income tax. Requires individuals who are obligated to make IDEA loan repayments to file income tax returns for the applicable years.

Constitutional Amendment - Requires the Congress, prior to each fiscal year, to adopt a statement in which total Federal outlays (except those for repayment of debt principal) do not exceed total receipts (except those derived from borrowing), unless a three-fifths vote of both Houses authorizes a specific excess. Limits the rate of increase in receipts in the statement to that of the increase in national income in the previous calendar year, unless law is enacted solely to approve specific additional receipts. Directs the President to submit a balanced budget. Authorizes waiver of these provisions in time of war. Sets a permanent limit on the amount of Federal public debt, prohibiting any increase unless legislation enacted by a three-fifths majority of both Houses becomes law.

Individuals With Disabilities' Trained Animals Parity Act of 1991 - Amends Federal law to allow as a benefit to certain veterans with service-connected disabilities the provision of an animal trained to assist individuals with the disability. (Present law encompasses only the provision of seeing-eye or guide dogs trained for the aid of the blind.) Makes similar changes (from "guide dogs" to "animals") to provisions requiring admission to public buildings and carrier rate charges for disabled individuals accompanied by assisting animals.

Rural Physicians' Incentives Act of 1991 - Amends the Internal Revenue Code to allow a deduction on medical education loan interest of a physician which accrues while the physician is living in and providing primary care to residents of a medically underserved rural area. Amends title XVIII (Medicare) of the Social Security Act to exempt services furnished in a rural area from a special fee schedule applicable to new physicians. Amends the Higher Education Act of 1965 to extend beyond the normal two-year limit, for borrowers serving an internship or residency program in preparation for practice in an area of primary care, the deferral of payments on: (1) insured student loans (Stafford Loans), including those eligible for interest subsidies; and (2) low-interest student loans (Perkins Loans). Amends titles XVIII (Medicare) and XIX (Medicaid) of the Social Security Act to modify provisions setting forth requirements concerning billing by one physician for services rendered by another physician in specified circumstances.

Rural Health Statistics Act of 1991 - Amends the Public Health Service Act to require that the geographic area description and analysis in a currently-mandated annual report on the health of the Nation's people include a description and analysis by rural area and urban area.

Rural Medical Emergencies Air Transport Act of 1991 - Amends the Public Health Service Act to direct the Secretary of Health and Human Services to make grants to at least seven States to assist in the creation or enhancement of air medical transport systems that provide victims of medical emergencies in rural areas with access to treatments. Requires: (1) preference for States with plans to integrate the proposed air medical system into a comprehensive network or plan for regional or statewide emergency medical services; and (2) the grant to be administered by the State agency responsible for medical emergency or trauma services programs. Allows the State to award grant funds to public and private entities. Authorizes appropriations.

Rural Mental Health Initiatives Act of 1991 - Amends the Public Health Service Act to establish, in the National Institute of Mental Health, the Office of Rural Mental Health. Mandates grants for demonstration projects to improve the availability of rural mental health services through providing such services in the same facilities as other health or social services, and otherwise integrating mental health, other health, and social services. Requires reports to the Congress. Requires that funds be made available to carry out these provisions from amounts appropriated under the Act for mental health research. Requires State comprehensive mental health services plans to describe how mental health services will be provided to rural residents.

Amends the Omnibus Budget Reconciliation Act of 1987 to allow a consortium including one or more rural hospitals (including at least a small rural hospital and/or one or more rural health clinics) to participate in the rural health medical education demonstration project of the Department of Health and Human Services. Authorizes appropriations as necessary to pay costs of such project which are attributable to travel and living expenses of medical residents and travel expenses of supervising physicians, to the extent such costs are not reimbursed under the Medicare program.

Rural Access to Obstetrical Care Act of 1991 - Requires the Secretary of Health and Human Services to: (1) provide for demonstration projects to reduce infant mortality by improving access in rural, underserved areas to obstetric services for eligible pregnant women under title XIX (Medicaid) of the Social Security Act; (2) develop and make available to the public each year a compendium of State initiatives to address the obstetric access crisis in rural areas; and (3) provide funding for a study, and report to the Congress on, obstetrical malpractice claims payment by Medicaid and private insurance companies.

Primary Care Training Amendments of 1991 - Amends provisions of the Public Health Service Act authorizing grants and contracts for training, traineeships, and fellowships in general internal medicine and general pediatrics to require that a school, hospital, or other entity, in order to receive a grant or contract, provide certain types of training related to family medicine. Adds demonstration of a commitment to family medicine to the criteria for establishing priority in making the grants and contracts.

Improving Access to Health Care Through Telecommunications Act - Mandates grants to rural health care consortia for acquisition and operation of interactive telecommunications systems to improve delivery of health care services. Allows information uses to include consultations, x-ray transfers, professional education programs, patient record transfers, and claims processing. Limits grants to one consortium per State, subject to waiver for consortia serving two or more States. Authorizes appropriations.

Antiterrorism Act of 1991 - Amends the Federal criminal code to define the term "international terrorism" to include activities that: (1) involve violent acts that are a violation of Federal or State laws, or that would be a criminal violation if committed within the jurisdiction of the United States or of any State; (2) appear to be intended to intimidate or coerce a civilian population, influence the policy of a government by intimidation or coercion, or affect the conduct of a government by assassination or kidnapping; and (3) occur primarily outside U.S. territorial jurisdiction or transcend national boundaries. Authorizes any U.S. national injured in his person, property, or business by reason of an act of international terrorism to bring a civil action in U.S. district court and recover treble damages and the cost of the suit, including attorney's fees. Specifies that a final judgment or decree rendered in favor of the United States in certain classes of criminal proceedings (such as those involving the murder of a foreign official, kidnapping, hostage taking, killing of a U.S. national, or an aircraft piracy-related offense) or in favor of any foreign state in a criminal proceeding to the extent that such judgment or decree may be accorded full faith and credit under U.S. law shall estop the defendant from denying the essential allegations of the criminal offense in a subsequent civil proceeding under this Act. Sets forth provisions with respect to jurisdiction and venue for, and limitation of, such civil actions.

Title I: Preconditions for the Receipt by China of Nondiscriminatory Treatment - Prohibits the President from recommending for a 12-month period in 1992 continuation of a waiver of human rights and emigration requirements for nondiscriminatory treatment (most-favored-nation treatment) for China under the Trade Act of 1974 unless a specified report is submitted to the Congress stating that China has accounted for and released prisoners who dissented in Tiananmen Square on June 3, 1989 and made progress in: (1) preventing gross violations of human rights (including in Tibet); (2) ending religious persecution and releasing members of religious groups who were detained for their religious beliefs; (3) removing restrictions there and in Tibet on freedom of the press and on broadcasts by Voice of America; (4) terminating harassment of Chinese citizens in the United States, including the return and renewal of passports confiscated for prodemocracy activities; (5) ensuring access of international human rights monitoring groups to prisoners; (6) ensuring freedom from torture and from inhumane prison conditions; (7) terminating prohibitions on peaceful assembly and demonstration imposed after June 3, 1989; and (8) adhering to the Joint Declaration on Hong Kong. Requires the President, if he recommends such extension, to include in a specified document to be submitted to the Congress a report on China's progress in implementing the above-mentioned measures.

Expedited Consideration of Proposed Rescissions Act of 1991 - Amends the Congressional Budget and Impoundment Control Act of 1974 to require a special message, in the case of budget authority proposed to be rescinded or reserved, to include language amending the law authorizing such programs to allow them to continue to function at the proposed new level of budget authority. Allows the President to transmit to both Houses of the Congress, for expedited consideration, one or more special messages proposing to rescind all or part of any item of budget authority provided in an appropriation bill. Requires that such special message be transmitted not later than three days after the President approves the appropriation bill and be accompanied by a draft bill or joint resolution that would, if enacted, rescind the budget authority proposed to be rescinded. Sets forth House and Senate procedures for the expedited consideration of such a proposal.

Amends the Export-Import Bank Act of 1945 to prohibit, except under specified circumstances, the Export-Import Bank from guaranteeing or extending credit for the sale of defense articles and services to any country (currently, only to economically less developed countries). Requires the Comptroller General to study the participation of the Bank in financing sales of such items to foreign countries and to submit the results of such study to specified congressional committees.

Amends the Financial Institutions Reform, Recovery, and Enforcement Act of 1991 to extend the phase-in period for State licensed appraisals. Prohibits the Appraisal Subcommittee from establishing a minimum experience requirement in excess of 500 hours as a licensing standard or qualification for State licensed appraisers.

Amends Internal Revenue Code provisions relating to the income tax deduction for the health insurance costs of self-employed individuals to: (1) increase the allowable deduction from 25 percent to 50 percent; and (2) make the deduction permanent (under current law it will expire after tax year 1991).

Expresses the sense of the Congress that: (1) the Secretary of State and the Director of the Peace Corps should formally inquire as to the interest of the Government of South Africa and other interested parties in such country in having a Peace Corps program in South Africa; (2) the President should initiate such a program if interest exists, provided that the President determines that continued progress is being made toward the abolition of apartheid and toward democracy in South Africa; and (3) such a program should provide basic educational and health services and teacher training in the black and colored townships.

Amends title XIX (Medicaid) of the Social Security Act to prohibit the Secretary of Health and Human Services from approving or renewing a waiver under a State Medicaid program of Medicaid's home health care exclusion if such program requires a patient to be or have been an inpatient of a hospital or a resident of a nursing facility in order to be eligible for home health care services.

Requires the display of the National League of Families POW/MIA flag: (1) at all national cemeteries and the National Vietnam Veterans Memorial on May 30, 1991 (Memorial Day), September 20, 1991 (National POW/MIA Recognition Day), and November 11, 1991 (Veteran's Day); and (2) on, or on the grounds of, the White House, and the buildings containing the primary offices of the Secretaries of State, Defense, and Veterans Affairs, and the Director of the Selective Service Commission on September 20, 1991 (National POW/MIA Recognition Day).

Expresses the sense of the House of Representatives that: (1) Japan has not fully complied with the 1986 U.S. - Japan Semiconductor Trade Agreement and that such failure has had an adverse effect on the national security and economic future of the United States; (2) the United States and Japan should enter into a new agreement that will open the Japanese semiconductor market and will address the threat of future dumping of semiconductors by Japan; and (3) the President, the United States Trade Representative, and the Secretary of Commerce should take all steps necessary to negotiate a strong, effective semiconductor trade agreement with Japan.

Rural Telephone Cooperative Associations ERISA Amendments Act of 1991 - Amends the Employee Retirement Income Security Act of 1974 (ERISA) to provide that rural telephone cooperative association employee welfare benefit plans shall be treated the same as those of rural electric cooperatives, for purposes of ERISA preemption of State laws (by excluding such telephone organizations from the definition of multiple employer welfare arrangements, which are subject to certain State regulations).
