Requires that every obligation of the United States be issued in registered form. Amends the Internal Revenue Code to prohibit the issuance in bearer form of securities which are interests in U.S. Government-backed securities. Imposes a three percent excise tax on the issuer of registration-required U.S. Government-backed securities which are not issued in registered form. Provides that the issue price of any U.S. Government-backed security shall be treated as the principal amount of the obligation. Imposes a 30 percent withholding tax on interest received by foreigners from U.S. Government-backed securities.

Permits the transmittal of alternate budget proposals which, if implemented, would result in a deficit or surplus if the President determines that a balanced budget is inappropriate for any fiscal year. Urges the President to submit to the House and the Senate, not later than September 30, 1984, a comprehensive plan to balance the Federal budget. Requires the Committee on the Budget of each House to report, by April 15, 1985, and by April 15 of each year, a balanced budget for the upcoming fiscal year. Requires the chairmen of the budget committees and an individual designated by the President to meet from time to time and agree upon common economic and technical estimating assumptions which shall be used for the preparation and reestimation of the President's budget. Establishes a commission which shall: (1) review, with respect to FY 1986, the balanced budget proposal submitted by the President and the budgets reported by the Committees on the Budget; (2) study the effectiveness of the provisions of this Act in achieving balanced Federal budgets; and (3) make recommendations for further changes which may be necessary to achieve balanced Federal budgets. Requires the commission to transmit to the President and Congress a report not later then December 31, 1986, containing a detailed statement of its findings, conclusions, and recommendations for such legislation and administrative actions as it considers appropriate. Terminates the commission 30 days after the submission of such report.

Money Laundering Penalties Act of 1984 - Amends the Currency and Foreign Transactions Reporting Act to increase the civil penalty for willful violations of the Act by financial institutions from $1,000 to $10,000. Increases the criminal fine from $1,000 to $250,000 and imprisonment from one year to up to five years. Increases from $5,000 to $10,000 the value of monetary instruments exported from or imported into the United States above which a person is required to file a report. Grants customs officers authority to make warrantless searches for evidence of violations of such Act. Allows the Secretary of the Treasury to pay rewards to persons who provide information about violations of the Act which lead to the collection of a criminal fine, civil penalty, or forfeiture of currency of more than $10,000.

Records and Reports on Monetary Instruments Transactions Amendments of 1984 - Increases the threshold reporting level for foreign currency transactions from $5,000 to $10,000. Makes it a Federal offense to attempt to transport or to attempt to have transported currency or monetary instruments outside the United States without filing required reports. Permits a customs officer to stop and search, without a search warrant, a vehicle, vessel, aircraft, or other conveyance, envelope or other container, or person entering or departing from the United States on which or whom the officer has reasonable cause to believe a monetary instrument for which a report is required is being transported in violation of the law. Increases the civil and criminal penalties imposed upon domestic financial institutions for violation of regulations concerning records and reports on domestic and foreign monetary instrument transactions. Permits the Secretary of the Treasury to pay a reward to an individual who provides original information leading to recovery of a criminal fine, civil penalty, or forfeiture exceeding $50,000 for a violation of such reporting requirements.

Pharmaceutical Assistance to the Aged Act - Amends title XIX (Medicaid) of the Social Security Act to permit a State plan for medical assistance to provide assistance for outpatient prescribed drugs and other items for individuals who are 65 or older and who are U.S. citizens in the United States or resident aliens and who have incomes below a certain level and are not otherwise eligible for medical assistance under the State medical plan if: (1) the State has in effect a law permitting a licensed pharmacist to substitute a less expensive interchangeable drug for another drug unless a licensed physician provides that a substitution is medically inappropriate; and (2) the State plan provides that the amount of payment will not exceed the cost of the least expensive interchangeable drug. Sets forth requirements governing amounts of payments. Directs the Secretary of Health and Human Services to provide for informing individuals who are 65 or older and who are entitled to Medicare benefits of the benefits available under this Act in States which have elected to have a program of pharmaceutical assistance for the aged pursuant to this Act. Requires the Secretary to promulgate final regulations by September 1, 1985, to provide for State implementation of this Act. Provides that this Act shall apply to medical assistance furnished with respect to outpatient prescribed drugs furnished on or after October 1, 1985. Directs the Secretary to maintain statistical records on State programs of pharmaceutical assistance to the aged to determine the effectiveness and impact of such programs. Directs the Secretary to report to Congress on the programs when the FY 1987 budget for the Department of Health and Human Services is transmitted to Congress.

Requires the Commission to: (1) identify problems which may hinder the control and reduction of Federal budget deficits; and (2) analyze all potential options which would result in deficit reductions and place the Government on a sound financial basis. Requires the Commission to report to the President and Congress by January 15, 1985, detailed findings, conclusions, and recommendations for such legislation and administrative actions as it considers appropriate. Terminates the Commission 30 days after submission of such report.

Medicare Participating Physician Act - Amends title XVIII (Medicare) of the Social Security Act to provide that: (1) in the case of a participating physician, payment under part B (Supplementary Medical Insurance) of title XVIII shall be determined according to provisions of this Act; and (2) payment cannot be made under part B to a new physician unless that physican is a participating physician. Provides that, in general, the amount of payment shall be: (1) during January 1985 in the case of a current physician, the monthly average payment made under part B for services furnished by that physician during 1984; and (2) during any subsequent month, the amount payable for the previous month increased (or decreased) by the amount by which the amount of payment for that previous month was less (or greater) than the amount that would otherwise have been payable (without regard to this Act) under part B with respect to physicians' services furnished by that physician during the previous month. Defines a: (1) "participating physician" as one who has entered into an agreement with the Secretary under which the physician will be paid under part B only in accordance with this Act and will not charge any amount not permitted to be charged under an assignment; (2) "current physician" as one who receives payments under part B before January 1, 1985; and (3) "new physician" as one who is not a current physician.

Medical Research Checkoff Act of 1984 - Amends the Internal Revenue Code to allow taxpayers to designate one dollar of their income tax payments to be paid over to the National Institutes of Health Medical Research Trust Fund, to be established in the Treasury by this Act. Appropriates to such trust fund an amount equal to the amount designated on tax returns. Requires the Secretary of the Treasury, after consulting with the Secretary of Health and Human Services, to establish within the trust fund an account for each institute established within the National Institutes of Health.

Alzheimer's Disease and Related Disorders Treatment Act of 1984 - Directs the Secretary of Health and Human Services to designate 20 centers for the treatment of Alzheimer's disease and related disorders. Requires such centers to: (1) provide a central facility for diagnosis, evaluation, and counseling; (2) develop standards of treatment and methods of delivering cost-effective services; (3) train professionals and nonprofessionals in diagnosing and treating such disorders; and (4) develop a comprehensive acute and long-term treatment program. Authorizes FY 1985 through 1989 appropriations. Directs the Secretary to make $1,000,000 available to each center for each of at least five years for teaching, training, research, and staffing. Permits eligible persons to elect to receive benefits as provided for by this Act rather than under titles XVIII (Medicare), XIX (Medicaid), or XX (Grants to States for Service) of the Social Security Act, or under the Older Americans Act of 1965.
