Summary
Muudatused lihtsustavad ultraharuldaste haigustega inimeste ravile pääsu, teevad apteegis valmistatavad ravimid haavatavatele rühmadele rahaliselt jõukohasemaks, samuti korrastatakse välisravi reeglid. Ultraharvikhaiguse ravimite rahastamise otsustamine muutub kiiremaks. Senine praktika sõltus sageli tootja ärihuvist ja taotlusest, mis võis võtta kuni 13 kuud. Üliharuldaste haigusega patsiendid ja nende arstid saavad edaspidi taotleda ravimi rahastamist erandkorras ise, kui tootja ei ole veel esitanud taotlust või ei teegi seda turu väiksuse tõttu. Tervisekassa hindab ravimi mõju, võtab tootjaga ühendust ja lepib tootjaga hinna kokku. Vajadusel konsulteerib ravimikomisjoniga. See võimaldab tervisekassa juhatusel teha otsuse kiiremini. Ravi tõenduspõhisusest põhimõttest ei loobuta. Väga haruldaste haigustega patsientide arv on väike, kuid kulud on suured ja mõjutab inimeste elusid. Muudatusega saab aastas kiiremini hüvitada ca 3–5 uut ultraharvikhaiguse ravimit kuni 10 patsiendile. 2024. aastal vajas ultraharvikhaiguse ravimit 55 inimest, kogumaksumusega 6,5 miljonit eurot. Teise olulise muudatusena saab Tervisekassa hakata standardretsepti alusel apteegis valmistatavaid ravimeid soodsamalt hüvitama. Seni maksid patsiendid apteegis valmistatavate ravimite eest täishinna. Soodusmäär hakkab kehtima eelkõige lastele ja neelamisraskustega inimestele. Standardretseptide loomine teeb ka arstidele ravimi väljakirjutamise ja apteekritele valmistamise kiiremaks. Seletuskirja kohaselt kirjutas eelmisel aastal apteegis väljastatavaid ravimeid välja ligikaudu 1500 arsti ning neid valmistas 112 apteeki. Vähemalt ühe apteegis valmistatava ravimi ostis 17 512 inimest, lahuste või pulbrite kasutajaid on ca 700. Kolmandaks, seaduse muudatustega ühtlustatakse ka välisravi ja riigisisese ravi korraldus ning muudetakse asjassepuutuv menetlus kõigi osapoolte jaoks kiiremaks ja õigusselgemaks. Välisravi on ravi või uuring välisriigis, kui Eestis vajalikku ravi, oskust või tehnoloogiat ei ole. Eelmisel aastal sai Tervisekassa kaudu välisravi 64 inimest. Eelnõus esitatud muudatuste jõustumisel väheneb inimeste, ettevõtete ja riigi halduskoormus. Muudatused on kavandatud jõustuma 1. mail 2026, et saaks teha vajalikud registri- ja infotehnoloogilised arendused.
