Après la référence : « L. 1434‑12 », rédiger ainsi la fin de l’alinéa 5 : « ou par des établissements et services de santé, sociaux et médico‑sociaux. L’agence régionale de santé sera compétente pour coordonner et accompagner les initiatives de projets territoriaux de santé. L’élaboration d’un projet territorial de santé est initiée, soit par une communauté professionnelle territoriale de santé ou soit par un établissement ou un service de santé, social ou médico‑social et dont le projet de santé a été approuvé  par l’agence régionale de santé. »
