   Le transport sanitaire héliporté occupe une place désormais incontournable dans l'organisation de l'offre de soins de médecine d'urgence. Avec 55 bases sur le territoire national, l'héliSMUR permet un accès à des soins urgents, en moins de 30 minutes, à de nombreux territoires éloignés ou difficiles d'accès en véhicule terrestre. A la différence des flottes héliportées d'État (armées, sécurité civile), les groupements d'établissements de santé passent des contrats avec des opérateurs privés, qui prévoient une prestation complète incluant l'hélicoptère, l'équipage (pilote et assistant de vol), ainsi que la maintenance des appareils. Les pilotes et assistants de vol sont ainsi recrutés par ces opérateurs privés auxquels incombe le dialogue social. Dans le cadre des procédures de commande publique avec ces opérateurs, les établissements de santé exigent le respect de la convention collective nationale (cahiers des clauses administratives particulières et techniques particulières). Depuis 2023, le Gouvernement a augmenté le budget dédié aux héliSMUR, de près de 52 % en 3 ans, pour faire face à l'augmentation des coûts spécifiques au domaine héliporté, ainsi qu'à la mise en œuvre d'évolutions réglementaires et opérationnelles. Toutefois, face aux situations de blocage du dialogue social au sein des entreprises d'héliSMUR, le ministère chargé de la santé et le ministère des transports ont missionné des médiateurs afin de permettre un dialogue entre les parties. Sous l'égide du ministère de la santé s'est tenue une réunion avec les syndicats de pilotes, les employeurs, le ministère des transports et les médiateurs pour faire le bilan des réunions de médiations. C'est ainsi qu'un accord a été trouvé le 30 juillet 2025 entre les syndicats de pilotes et leurs employeurs. De nouvelles perspectives de travail, structurantes pour mener un travail de fond sur le transport sanitaire héliporté, seront organisées par le ministère de la santé.
