Its frequency has increased significantly over the past two decades with the disease expected to affect 592 million adults worldwide by 2035. Some medications are well known, effective and inexpensive. Metformin improves the sensitivity of cells to insulin but can cause digestive problems in certain patients. Its use is not recommended when the kidney is functioning poorly and must be discontinued in acute situations such as a vascular accident, infection or surgery. Sulfonamides and glinides, two classes of drugs with a very similar mode of action, stimulate the production insulin through the pancreas. But they can cause hypoglycemia, which means that the patient's blood sugar level must be checked very often. In addition, sulfonamides are contraindicated in cases of renal insufficiency. Acarbose, which slows down the intestinal absorption of glucose and helps avoid blood sugar peaks after meals, can cause digestive problems. Therapeutic treatments with newer drugs, such as DPP4 inhibitors and GLP-1 analogues, provide solutions to patients suffering from diabetes but remain very expensive and their duration of action is very short (particularly with regard to GLP-1 because it is destroyed by the DPP-4 enzyme). The advantage of these new treatments is based on the fact that they have benefited from cardiovascular safety studies as part of their development. Some of them also have a protective effect on the cardiovascular level, which is an additional argument for their use. The MP questioned the Government in September 2017 about this ethnic group of Muslim religion which suffers persecution and abuses which push tens of thousands of them to flee to neighboring countries (Bangladesh, Malaysia, Thailand). Over the past two years, around 740,000 people, the majority of whom are children, have been forced to move. Non-governmental organizations have reported executions of civilians by the armed forces and acts of torture and denounced the presence of anti-personnel mines placed on the routes of this mass exodus. With regard to international law, these acts which constitute an offensive systematic and large-scale attacks against the Rohingyas constitute crimes against humanity, as does the system of discrimination and segregation put in place by the State which is akin to apartheid. Investigations are currently being carried out by the United Nations Human Rights Council. However, the lack of cooperation from the Burmese government renders the judicial process ineffective, even though senior officials are involved in these abominable acts. At the Security Council on February 28, 2019, as well as at the 40th session of the Human Rights Council in March 2019, France expressed strong recommendations, such as the immediate establishment of safe and unimpeded humanitarian access in Arakan State or the assurance that internally displaced Rohingya people can have full freedom of movement, which we can only greet. However, this remains insufficient given the scale of this tragedy. To get out of this impasse, it would be desirable to open up the possibility for orthoptists to renew or adapt optical corrections under the same conditions as eyewear opticians. The orthoptists, already authorized to carry out screenings, assessments and to take charge of rehabilitation, have all the skills necessary for this purpose. In the current state of the regulations, orthoptists can carry out a visual assessment as part of the renewal or adaptation of optical corrections. However, the prescribing doctor remains solely competent to write the prescription corresponding to the orthoptic diagnosis. A such organization is not effective. It mobilizes several healthcare professionals, involves numerous return trips and generates unacceptable additional financial costs for the patient and health insurance. The reorganization of the sector mentioned here would make it possible to relieve congestion in ophthalmology practices, reduce the cost of orthoptist consultations and promote prevention in visual health. This solution would have the merit to be in line with the health system transformation plan announced by the President of the Republic and the latest recommendations from the Court of Auditors. In its latest report on the application of social security financing laws, the Court clearly called for a modification of the distribution of skills between the different actors in the visual sector and, more specifically, for the granting of authorization for orthoptists to carry out visual assessments and simple consultations and to prescribe optical equipment. This profession is accessible after university training attested by a certificate of ability as an orthoptist, the organization and content of which are set by a decree of October 20, 2014. This course provides for several weeks of internship during the six semesters of study. Internships can in principle be carried out in public or private hospital, medico-social, educational structures and in private practices. Several points of the training grid have, however, never been specified: the status of the private internship supervisor, the homogeneity of training throughout the territory, the means of controlling the authorizations of training places and the orientation of practical teaching. These regulatory uncertainties have seriously penalized orthoptists. With particular regard to practical teaching, it appeared that internships in private practice were essentially devoted to assistance during ophthalmological consultations. Rehabilitation and rehabilitation missions, which nevertheless constitute the heart of the profession, found themselves marginalized or even excluded from training. Orthoptics students now favor internships in ophthalmology practices, with better defined contours and accompanied by remuneration. During discussions on the social security financing bill for 2019, this difficulty was raised and was linked to the prohibition provided for in paragraph 3 of article L. 4381-1 of the public health code, according to which "The completion of these internships cannot have the aim or effect of increasing the paid activity of these practitioners. This provision was interpreted as prohibiting orthoptist students from carrying out internships with a privately practicing orthoptist. Several amendments were tabled to delete this paragraph. They were supported by the rapporteurs of the social affairs committee of each chamber. However, the Government opposed this deletion, considering that the provisions referred to did not have the effect of prohibiting internships with independent orthoptists but only the invoicing of their interventions with patients. Paragraph 3 of Article L. 4381-1 of the Public Health Code therefore remained unchanged. Everyone is aware that this need will continue to increase inexorably with the “grandpa-boom” generation over the next few years. years. Unfortunately, this profession is less and less attractive and these personnel must face the deterioration of their working conditions. According to the CRDLA report dated March 2016 on the state of the home help sector and its needs, their salary amounts to barely 932 euros while the monthly salary of the French is 2,250 euros for all genders and socio-professional categories combined. It has not been revalued since years and their purchasing power has fallen by 13% in 10 years. Added to this are their mileage expenses to travel to patients’ homes. They travel many kilometers during the day, particularly in rural areas, and their mileage allowance amounts to only 0.35 cents/km (including 0.13 euros for fuel). In addition, home helpers who take sick leave, very often due to working conditions, hard work and incompatible with family life, are difficult to replace. This problem is increasing in cross-border areas such as the constituency of the MP which experience a strong disparity in salaries between France and Luxembourg which remains more attractive. The UN Secretary General also spoke of “ethnic cleansing” when referring to this human tragedy. A founding member of the United Nations in 1945, France, which plays a leading role in subjects as crucial as that of respect for human rights, cannot remain indifferent to this massacre. It is its duty to put an end to the abuse against this population with other UN member states. While “natural” medicines are on the rise, the use of this plant is controversial in France. While La Maison de l'artemisia, a French association, claims that the plant, taken in the form of herbal tea, would treat and prevent disease even more effectively than conventional treatments, French doctors and researchers denounce this practice for which evidence of effectiveness is currently lacking. “Artemisia annua” is however well known to scientists and in particular artemisinin, a molecule capable of eliminating the parasite responsible for malaria, which is extracted from it. This discovery also earned the Nobel Prize for Chinese medicine Tu Youyou in 2015. Since then, artemisinin has been an essential compound in anti-malaria drugs, ACTs (therapeutic combinations based on artemisinin), so much so that 28,000 hectares are put into semi-industrial cultivation around the world to supply the pharmaceutical industry.
