Après l’alinéa 7, insérer l’alinéa suivant : «  j ) L’article L. 5125‑1‑1 A, afin de permettre aux pharmaciens d’officine, en dehors du cadre des coopérations prévues à l’article L. 4011‑1, d’être désignés comme correspondants au sein d’une équipe de soins par le patient, à la demande ou avec l’accord du médecin traitant, et ainsi de renouveler des traitements chroniques ou d’ajuster leur posologie. Le médecin traitant et le pharmacien correspondant sont signataires d’un projet de santé au sens des articles L. 6323‑3, L. 6323‑1‑10, L. 1411‑11‑1 ou L. 1434‑12 du code de la santé publique. »
